I take this song to be about unrequited* love that she is unable to confess or confront even though it is ruining her psychologically, destroying her self-image and causing her to blame herself for her suffering
*it seems like it's also possible that she doesn't know how the other person feels about her, but she assumes it is unrequited love in order to avoid confrontation/ rejection
